1. How do I schedule my first appointment? Please call the office at 937-392-4318, Ext. 234, to speak with the Office Manager who will schedule your appointment.
2. How far in advance should I make my appointment?
We do our best to schedule your first appointment within one week of your request.
3. Is there anything I should do to facilitate my first appointment?
If at all possible, please arrive 15 minutes early to your first appointment and bring all required Insurance and Medical Cards. Please bring your prescription for therapy to your first visit and present it when you sign in at the Reception Desk. 4. What is the policy of canceling appointments?
If you arrive more than 15 minutes late for your appointment, your therapy session will need to be rescheduled. If you are “no show” to your appointment, you will be charged a $25.00 fee which is payable at your next scheduled appointment. A “no show” is when a patient does not show up for the scheduled appointment time with no prior communication that he/she will not be fulfilling his/her scheduled appointment time. If you need to cancel an appointment, we ask that you give us 24 hours notice whenever possible.
5. When are Co-Payments due?
Co-payments are due at the time of your appointment. Cash or check is acceptable.
6. Who is responsible for charges not covered by my health plan?
You are responsible for all charges not covered by your health plan. Our billing department will bill your insurance and will then send you a bill for any remaining charges.
7. Whom do I call if I have a question regarding my bill or my insurance claim?
Please call 937-392-4318 and ask to speak to our Accounting Department.
8. Do you accept new patients?
Advanced Regional Therapy is always accepting new outpatients.
9. Do you accept children and young adults?
Advanced Regional Therapy provides services for patients from infants through geriatrics. We will meet your rehabilitation needs, no matter what your age. |
